10/13/2023 EnchantaVaughn Approved Behavior: QMHP-A greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her evening. Ms. Brown verbalized to QMHP-A that her evening was going well. QMHP-A informed Ms. Brown that this session was going to continue to focus on managing her depressive symptoms. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHP-A began the session by inquiring about Ms. Brownâs current thoughts, feelings, and emotions. QMHP-A engaged Ms. Brown in a skills building activity at the beginning of the session. QMHP-A explained to Ms. Brown that the purpose of this activity was to assist Ms. Brown with identify emotions which she experiences frequently utilizing current and past emotions that causes her to experience depression and other psychological issues. QMHP-A engaged Ms. Brown in discussion surrounding triggers of emotions. QMHP-A challenged Ms. Brown to identify a trigger in which could arise emotion. QMHP-A engaged Ms. Brown in another skills building activity. QMHP-A explained to Ms. Brown that the purpose of this activity was for Ms. Brown to identify positive benefits of communicating feeling, indicate possible negative consequences of communicating feelings, and rank statements in order of importance. Once the activity was completed QMHP-A reviewed the activity with Ms. Brown and allowed Ms. Brown to see where majority of her anger and frustration is streaming from and ways to possibly prevent that anger from occurring. Response: Ms. Brown appeared to be receptive to QMHP-Asâ interventions and services that were provided to her during the session. Ms. Brown indicated to QMHP-A that she is overwhelmed with the whole situation at hand and just wishes that a miracle would happen in her life right now. Ms. Brown verbalized to QMHP-A that she does not have any thoughts of wanting to harm herself but she has thoughts of wanting to harm those who have caused her to be in the position that she is currently in now. She expressed to QMHP-A that she needs to just stop trying to help everyone out and she has definitely learned her lesson about helping others out and taking care of herself because others do not think about her. Plan: QMHP-A will teach the client relaxation skills (e.g., progressive muscle, guided imagery, slow diaphragmatic breathing) and how to discriminate better between relaxation and tension; teach the client how to apply these skills to her daily life. (1-2x weekly).
